Paperwork and Documents

Congratulations and welcome to the AVID Technical Resources Team! Now that you have accepted a contract position, you will need to fill out a few forms. Please contact your AVID IT recruiter if you have any questions. An AVID Customer Support Representative will also be assigned to you to help answer any questions and make your transition as seamless as possible.

Here is a list of the mandatory state and federal documents as well as some additional forms to help get you set up on the AVID payroll:

  • AVID Technical Resources Time Card

    This is the timecard which you will need to complete every week when reporting your hours. Please fill out this form?and send it back to your AVID IT recruiter or dedicated AVID Customer Support Representative.

  • Direct Deposit Form (optional)

    Complete and return this document if you want your paychecks deposited directly into your bank account. Please fill?out this form and send it back to your AVID IT recruiter or dedicated AVID Customer Support Representative.

  • Expense Form

    This expense form is to be used in case you incur expenditures on the job that are reimbursable back to you. Please?remember to keep all receipts and send (or fax) the copies back to your AVID IT recruiter or dedicated AVID Customer?Support Representative along with a completed expense form.

  • I-9 (Employment Eligibility Verification)

    This form is to verify that you are legally eligible to work in the United States. You will need the appropriate proof of?identification as outlined in the I-9. An AVID IT recruiter or your dedicated AVID Customer Support Representative will?need to verify this in person.

  • W-4 (Federal Tax Form)

    This is the form used to accurately calculate how much tax to withhold from your paycheck. Please fill out this form?and send it back to your AVID IT recruiter or dedicated AVID Customer Support Representative.

  • M-4 (or appropriate State Tax Form)

    If you are working in Massachusetts, you will need to complete an M-4. Most other states have their own tax form?(note: Some do not have their own state tax document. In these cases, your state withholdings will default to what?you list on your W-4). Please complete this form with your information and send it back to your AVID IT recruiter or?dedicated AVID Customer Support Representative.

  • Medical Emergency Contact Information

    Please fill out the Medical Emergency Contact information so that we have someone to contact if something were to?happen to you. This is a mandatory form. Please fill out this form and send it back to your AVID IT recruiter or?dedicated AVID Customer Support Representative.

  • Medical Insurance

    Please contact your AVID IT recruiter or dedicated AVID Customer Support Representative for more information and?details about our various medical insurance plans.

  • Dental Insurance

    Please contact your AVID IT recruiter or dedicated AVID Customer Support Representative for more information and?details about our dental insurance plans.

  • 401k

    There is also an opportunity to start a 401K with us. In order to be eligible for the 401K, you need to have been?working with AVID Technical Resources for at least 6 months. If you are eligible, please ask your AVID IT recruiter or?dedicated AVID Customer Support Representative for the appropriate forms.
